Membership Based Practice Policy
In order to continue to provide you with the high quality personal service you appreciate, we need to assess an Annual “Membership Fee”.
The Fee
$150/per patient/annually
OR
$350/family-unit/annually
Starting 02.01.2019
Annually February 1st through January 31st
Checks, credit and debit cards accepted
Why?
Nurse Practitioners are reimbursed at 20% lower rates than physicians
We have the same overhead costs as other offices
We see less patients per day in order to give you personalized care
We want to stay true to our vision and mission
Costs to NPHC
E-scribing your prescriptions to your pharmacy
Access to your patient portal
Sharon and Sandy work an additional 20-30 hours a week, before and after hours, on our days off, and often on weekends, to provide you with our personalized care. This includes refills, emailing test results, answering questions, reviewing consult notes and collaborating with your specialists.

Patient portal communication in the early morning hours, evenings, on weekends and often on holidays
When needed, we call you personally on the phone
Emergency line after hours where you can talk to the Nurse Practitioner on-call if it can’t wait till office hours
Registered nurse, to call and explain results as needed or triage your calls, along with many other features
